Patient Information

Please arrive one hour ahead of your scheduled surgery, unless otherwise instructed.
Please bring a photo ID, such as a drivers license and your insurance or Medicare card. You will be asked for your insurance co-pay or deductible payment unless prior arrangements have been made with our Patient Accounts Representative.
If you have other questions or concerns, please notify the nurse who helps prepare you for surgery. He or she will answer your questions or have your doctor visit you before you go to surgery.
After your surgery, when you are awake, a friend or family member may join you in the Phase II Recovery Area.
When you are fully awake and alert and it is safe to do so, a staff member will go over specific instructions for your at-home care and you will be discharged.
Upon discharge, a friend or family member must take you home and should stay with you for 24 hours. You will not be released to drive yourself home, if you have had general anesthesia, sedation or pain medication.
Due to space limitations, please limit the number of people who will accompany you to the Surgery Center: for children under age 18 please limit to both parents, for patients 18 or older, please limit to your ride home.
Do not eat solid food 6 hours prior to surgery, unless you have been given other instructions by your doctor or anesthesiologist.
You may drink water, apple juice, black tea and coffee up to three hours before surgery time.
Try not to smoke the day of your surgery.
Shower or bathe to minimize the chance of infection.
Do not wear make-up.
If appropriate, bring insulin or your inhaler with you.
Wear loose, comfortable clothing to avoid pressure on your surgery site.
Wear flat, preferably slip-on shoes.
Leave jewelry and valuables at home.
If you wear contact lenses, bring your lens supplies and glasses.
If you are on medication, check with the surgeon to see whether you should take it the night or morning before your surgery.
If you have any health changes prior to your surgery, such as minor elevations in temperature, a rash, a cold, or cough, contact your surgeon or the Surgery Center immediately.
If you suspect that you are pregnant, contact your surgeon; medications may be harmful to your developing child.
Arrange for someone to drive you home and stay with you for 24 hours.
Bring a list of all medications you are currently taking. We will also ask you about any allergies you might have.
For young children who are undergoing surgery, please bring a comforting toy, blanket, pacifier, or bottle (for after surgery). For older children under age 18, a parent or legal guardian must sign a surgery consent form. Legal guardians should bring appropriate documentation.
As a patient, you have the right to:
- Considerate, respectful care at all times and under all circumstances with recognition of your personal dignity.
- Personal and informational privacy, within the law.
- Information concerning your diagnosis, treatment, and prognosis, to the degree known.
- Confidentiality of records and disclosures. Except when required by law, you have the right to approve or refuse the release of records.
- The opportunity to participate in decisions involving your health care, unless contraindicated by concerns for your health.
- Make decisions about medical care, including the right to accept or refuse medical or surgical treatment and the right to initiate advance directives such as a living will or durable power of attorney. If you already have a living will or other directive or you wish to initiate one, please speak with a nurse.
- Information concerning implementation of any advance care directive.
- Impartial access to treatment regardless of race, color, sex, national origin, religion, handicap, or disability. (Allied Physicians Surgery Center adheres to all federal and state rules and regulations. Our policies promote a non-discriminatory environment for all of our surgical guests.)
- Receive an itemized bill for all services.
- Know the identity and professional status of individuals providing service to you.
- Report any comments concerning the quality of services provided to you during the time spent at the facility and receive fair follow-up on your comments.
As a patient, you are responsible for:
- Providing, to the best of your knowledge, accurate and complete information about your present health status and past medical history and report any unexpected changes to the appropriate practitioner(s).
- Following the treatment plan recommended by the primary practitioner involved in your case.
- Providing an adult to transport you home after surgery and an adult to be responsible for you at home for the first 24 hours after surgery.
- Indicating whether you clearly understand a contemplated course of action and what is expected of you.
- Your actions if you refuse treatment, leave the facility against the advice of the practitioner, and/or do not follow the practitioner's instructions relating to your case.
- Assuring that the financial obligations of your health care are fulfilled as expediently as possible.
- Providing information about and/or copies of any living will, power of attorney or other directives that you desire us to know about.
